This series documents the organization, operational functions, educational outreach, and patient services of the Duke Midwifery Service. Contains annual reports, awards, correspondence, grant applications, histories, meetings, organizational records, resident orientation materials, staff schedules, resources, and guidelines. Also included are the Midwifery birth logs for Certified Nurse Midwife assisted labor and deliveries at Duke University Medical Center and the Durham County Regional Hospital. The birth logs were used prior to the adoption of Obstetrics Trace View (OBTV) and Epic electronic record management systems. Some of these materials contain confidential patient information and are restricted under law by the Health Insurance Portability and Accountability Act (HIPAA). These include records of the officers of the University, as defined in the Bylaws, the deans of schools and colleges, and departments, institutes, and other offices as designated by the President. For a period of twenty-five years from the origin of the material, permission in writing from the director of the office of record and the Medical Center Archivist is required for use.
